Youth Support Worker

At Naza Connect Care, we are currently expanding out team and are seeking committed individuals with a passion for working with young people and empowering them to achieve their goals and aspirations in a consultative and caring way.

Job description/Criteria
Principle duties & responsibilities

ONLY APPLY IF YOU CAN WORK 'SLEEPING IN NIGHTS'... this is a sleeping in role.

From our Youth Support Workers we are looking for someone who can:

  • manage a caseload of 6 young people and preempt their support needs
  • support vulnerable young people and effectively deal with challenging behaviour from young people.
  • Someone that can create a positive homely environment where the young people feel cared for and supported with their needs
  • keep daily accurate records as a basis for regular monitoring and tracking progress of young adults. This will be achieved via log entries as well as other monitoring document
  • Prepare detailed high-quality weekly/monthly progress reports regarding each young person’s progress which is submitted to the placing authority within deadlines.
  • To adhere to identified plans of support including risk assessments.
  • To complete the ‘support session’ document when work has been undertaken or observed with residents in a variety of situations/circumstances i.e further education, training providers, potential employers.
  • To act as an advocate for young adults in a variety of situations/circumstances.
  • Ability to motivate individual or groups of young adults to take part in social and leisure activities, enabling them to develop personal and social skills.
  • Ability to deal with difficult, demanding and stressful situations appropriately and objectively.
  • To manage own time and tasks effectively i.e. complete all required tasks for each shift, including the daily tasks, weekly and monthly tasks and health & safety checks.
  • Ensure all residents files are well maintained i.e. returning items to the correct sections and comply with GDPR.
  • Ensure the cleaning duties of the day are carried out to a good standard.
  • To follow Policy and Procedure/Employee Handbook without deviation i.e. contact management in timescales outlines regarding sickness.

Salary & hours

Shift pattern: 24hr contract, 3 to 4 sleep in shifts a week

Between Monday to Sundays to 9am till 11pm then sleep-in (with breaks in between)

£22,000.00-£31,200.00 per year

Locations: any of our homes within the Redbridge Borough

  • commit to sleep-in night shifts
  • Proficient administrative skills
  • Excellent Communication skills both verbal and written
  • Aged 22+ due to nature of client group
  • MUST have valid enhanced DBS & Available for immediate start
  • MUST HAVE minimum level 3 social care qualification
  • MUST HAVE experience of working with young people / challenging behaviour
  • Must be able to drive or committed to learning

Service Support Coordinator

We are currently recruiting for service support coordinator, see below for more details. 

   

The service support coordinator plays an important role within Naza Connect Care’s management team, to proactively identify and work with placement support staff (key workers) and young people (YP’s) to provide coordination and navigation of the YPs support across key services. The service support coordinator will work closely with placement key workers across our support homes and assist with advice and guidance to help key workers manage their caseloads of YP’s, acting as a central point of contact to ensure appropriate support is being delivered to the YP’s and ensure their needs are being met. This is achieved by bringing together all the information about a YP’s identified support needs and exploring options to meet these needs within a single personalised support plan, based on what matters to the young person and developing independent living skills. To develop and build on existing pathways to encourage appropriate referrals or appropriate interventions or specialist services as and when required in conjunction with YP support plans. 


    

Responsibilities: 

Service user support 

  • Identify and carry out appropriate methods to engage with colleagues and service users to co-produce the design of the service support delivery.
     
  • Plan, co-ordinate and facilitate group and 1:1 sessions.
     
  • Conduct monthly caseload reviews with support workers to discuss the needs of the
    young people on their caseload and identify appropriate support.
     
  • Conduct weekly support session planning with support workers to ensure actions identified in the caseload reviews are being completed
     
  • Keep a record of advice & support offered to support works and share with service manager to assist with staff supervisions
     
  • Keep a record of tasks given to support works where were not completed and share with service manager to assist with staff supervisions
     
  • The service support coordinator will encourage high productivity and meet set goals.
     
  • Discuss with support workers any outstanding tasks identified by the Quality Compliance
    officer which needs completing.
     
  • Plan co-ordinate and facilitate group and 1:1 sessions with the young people to capture their views, comments or concern
     
  • Work closely with key workers to manage challenging behaviour and act as a mediator to resolve conflict and encourage effective communication between support workers and YPs when necessary
     
  • Support key workers to monitor and record patterns of behaviour, identify triggers and patterns and implement proactive positive behavioural support responses to manage and reduce behaviours.
     
  • Build strong relationships with young people and support workers be a positive, motivating and inspiring influence
     
  • Use appropriate methods of service delivery – face-to-face or video call.
     
  • To plan, co-ordinate and facilitate engaging activities and outings for the YPs. 

  

  • Manage a budget for activities and outing costs and keep a record of all transactions
     
  • Apply a person-centred approach in each YP case you support in
     
  • Support Key workers in the creation of a personal action plan.
     
  • Liaise with professionals on behalf of young people and advocate when appropriate.
     
  • To ensure that children and young people’s ideas, thoughts and feedback are recorded, responded to and actioned. Feedback should be shared with Service Manager.
     
    • Report any safeguarding concerns to the Designated Safeguarding Lead
       
    • To work in partnership with referring LA’s, stakeholders and key agencies to ensure provision of comprehensive support to children and young people
       

• To carry out reviews in partnership with children and young people and relevant partners 

• Comprehensively plan (with children and young people and key agencies) move on from the Service
• To maintain clear, children and young people friendly, written and verbal communication where appropriate 

• To ensure key workers maintain an accurate and up to date record of support provided to Children and Young people 

  • To ensure quality standards are maintained and improved upon
     
  • To work to targets as identified in monthly goals and contribute to the organisational strategic direction through annual team objectives
     

• To always remain focused on the ultimate goal of supporting staff so the young people are supported to build their independent living skills and achieve positive outcomes in health, social and education. 

  • To meet with the manager for monthly supervision
     
  • To engage in completion of annual training needs analysis and attend relevant training
     

• To develop new, innovative, and alternative approaches to working with children and young people and ensure we working in line with relevant Ofsted regulations 

  • To encourage children and young people to participate in reviews, house meeting
     
  • To ensure children and young people are aware of and have access to other Services provided by Naza Connect Care
     
  • To pro-actively engage in Service development and raising the profile of Service
     
  • Promote team cohesion and development through reflective practice, innovative thinking, transparent communication, and solution focused approach
